The Ayos Health Card is a full HMO plan providing comprehensive healthcare coverage, including Day 1 coverage for Pre-Existing Conditions (PEC) once activated. It offers convenient, affordable, and accessible protection for individuals, families, and companies.

Anyone 18–65 years old and 15 days–17 years old for minor dependents.
Foreign applicants may apply with a valid ACR.

Pre existing condition -*claims are always subject to assessment of our medical director as part of fair usage.

Medical underwriting is required upon application.

Online application and easy approval within 24-48- hrs, activation is only 15 days.

Benefits vary per plan and may include:

Effective date always falls on the nearest 1st day of the month from verified payment date.

Yes, YOU WILL RECEIVE A COC after 15 days, which you can use to avail our services.

You can use your plan at PhilCare-accredited hospitals, clinics, and medical facilities nationwide.

Pre existing limits are always subject for evaluation of our medical directors, illnesses that falls as comorbidity and complications from an existing illness will share one limit.

Yes. Outpatient consultations are included in selected Ayos Health Card variants. Please check your Schedule of Benefits.


Yes. Only an employee masterlist is required—no complicated corporate documents.


Yes. OFWs may buy the plan for themselves or their families.
